Goal: to present Hackethal’s method as an alternative treatment of humeral shaft fracturesResults: critical evaluation of Hackethal’s method as an alternative means of treatmentSummary: Hackethal’s method is a surgical procedure used for treatment of humeral shaft fractures. This method enables the patient to make unlimited movements with the injured limb directly after the operation and without plaster cast because it is sufficiently stabilized by Kirschner wires in order to create proper union of bone tissues. Hackethal’s method is an alternative to the traditional surgical fixation using plates and to the external fixation and it is becoming increasingly frequent in upper extremity surgery. The fractured upper limb is dissected in the elbow articulation above the olecranon fossa. In this location a square the size of 0,25cm2-1cm2 is cut out to open the medullar cavity of the humerus. Next, 4-5 Kirschner wires are applied through the opening and fastened under radioscopic control to the humeral head. The proper anatomical alignment of the bone fragments should be secured the whole time. After the fracture heals Kirschner wires are removed. Complications of the procedure are rare and include extensive bleeding, radial nerve damage (which is rather caused by the fracture itself than the procedure), delayed union or nonunion. Osteomyelitis practically does not occur. In conclusion, Hackethal’s method is an interesting procedure, which can successfully be performed as an alternative in treatment of humeral shaft fractures. |
